Blood sugar rarely changes in isolation. Weight, insulin resistance, blood pressure, cholesterol, liver health, kidney health, sleep, and cardiovascular risk often move together — and care that focuses only on the glucose number may leave much of the picture unexamined. At WellifyCare Medical, diabetes and prediabetes are evaluated and treated as part of your complete metabolic health.

Prediabetes and type 2 diabetes
Prediabetes and type 2 diabetes
Prediabetes describes blood sugar that is higher than normal but not yet in the diabetes range. It is common, often causes no symptoms, and is frequently identified only through testing. It can also represent an important opportunity because the trajectory may sometimes be changed with appropriate treatment and follow-up.
Type 2 diabetes develops when the body becomes less responsive to insulin and the pancreas can no longer fully compensate. Insulin resistance may be present for years before blood sugar rises enough to meet diagnostic criteria, which is part of why related metabolic risks can accumulate quietly.

Full medical history, including weight trajectory and family history
Review of current medications, including medications that may affect blood sugar
Assessment of blood sugar control and how it has changed over time
Review of available glucose data, including home glucose readings or continuous glucose monitoring data when clinically indicated
Evaluation of related cardiometabolic factors including blood pressure, cholesterol, liver health, kidney health, weight, and sleep
Screening for obstructive sleep apnea risk when clinically indicated

Weight-related treatment when appropriate

Because weight and glucose are closely connected, addressing weight may meaningfully improve metabolic health for many patients.

Medication when clinically appropriate
Medication when clinically appropriate
Several medication classes are available for type 2 diabetes, and treatment is selected based on blood sugar, weight, cardiovascular and kidney risk, other medical conditions, contraindications, preferences, and insurance coverage. Some medications may provide benefits beyond glucose lowering.

Glucose monitoring when appropriate
Glucose monitoring when appropriate
Home glucose monitoring or continuous glucose monitoring (CGM) may be used when clinically indicated to better understand glucose patterns, treatment response, and day-to-day variability. The type and duration of monitoring depend on diagnosis, medications, clinical goals, and insurance coverage.

Blood pressure, cholesterol, kidney function, liver health, weight, and sleep-related risk are followed alongside glucose rather than treated as unrelated issues.

Scope of care
Scope of care
WellifyCare Medical provides care for adults with type 2 diabetes and prediabetes within a physician-led Internal Medicine and Obesity Medicine practice. Some conditions are better managed by, or alongside, endocrinology and other specialists — including type 1 diabetes, diabetes during pregnancy, and complex cases requiring subspecialty management. When specialty care is appropriate, WellifyCare Medical will recommend and coordinate appropriate referral.

Weight and blood sugar
Weight and blood sugar
For many people with prediabetes or type 2 diabetes, weight and glucose are closely connected. Changes in weight can influence insulin sensitivity, and improvements in metabolic health may follow. This is one reason medical weight management and diabetes care fit naturally together within WellifyCare Medical rather than being treated as separate silos.
Protecting kidneys, heart, and liver
Protecting kidneys, heart, and liver
Diabetes and prediabetes can affect health beyond blood sugar. Kidney function may change gradually and without symptoms, and early detection can influence treatment decisions. Cardiovascular risk reflects the combined effects of glucose, blood pressure, cholesterol, weight, kidney health, and other factors. Metabolic liver disease can also occur alongside insulin resistance and may be silent. WellifyCare Medical evaluates these related risks as part of the broader metabolic picture.
