01 Book Your Assessment
Choose an available virtual or in-person appointment through our secure Elation scheduling page.
02 Complete Your Secure Intake
Before your visit, complete your medical history, medication list, prior results, home readings, and primary health goals through the secure patient system. Do not submit detailed medical information through the general website contact form.
03 Initial Physician Evaluation
Meet with Dr. Dere for an approximately 45–60-minute evaluation. Visit length depends on clinical need and complexity.
04 Individualized Testing When Needed
Testing is selected based on clinical need. Laboratory testing and other third-party services are billed separately by the providing vendors. Insurance may be used when accepted and applicable; coverage is not guaranteed.
05 Results and Roadmap Visit
Meet again with Dr. Dere for an approximately 45–60-minute results and roadmap visit. Visit length depends on clinical need and complexity. You will review the findings, priorities, and written Cardiometabolic Health Roadmap.
06 Choose Your Next Step
Your roadmap may support self-directed prevention, coordination with your primary care clinician or specialists, focused cardiometabolic follow-up, additional evaluation, or optional Connected Obesity Care. There is no automatic enrollment or membership requirement.
Payment and Eligibility
Connected Cardiometabolic Assessment may be eligible for payment using HSA or FSA funds; patients should confirm eligibility with their plan administrator. Medicare and Medicaid beneficiaries should contact WellifyCare Medical before booking to confirm eligibility.
Where Care Is Available
Virtual appointments are available to eligible adults physically located in Wisconsin. In-person appointments are available in Brookfield, subject to scheduling and clinical appropriateness.
Insurance Status
Insurance credentialing is in progress. Insurance-based services may become available after payer contracting and credentialing are completed. Availability, participating plans, and timing are not guaranteed.
