Initial Evaluation — $250
Your care begins with a comprehensive physician evaluation with Dr. Dere, available virtually or in person. This approximately 60-minute visit reviews weight history, previous treatments, medications, appetite and eating patterns, diabetes or prediabetes, blood pressure, cholesterol, liver and kidney risk, sleep apnea risk, nutrition, movement, strength, stress, prior tests, family history, goals, and treatment options. Clinically appropriate laboratory testing or additional evaluation may be ordered. The initial evaluation does not require enrollment. If you choose to enroll, a six-month commitment is required.
Medication may be considered when clinically appropriate after reviewing benefits, risks, contraindications, interactions, coverage, and cost. Options may include Wegovy injection or tablets (semaglutide), Zepbound (tirzepatide), Saxenda (liraglutide), Qsymia (phentermine/topiramate), Contrave (naltrexone/bupropion), orlistat, and phentermine for appropriately selected patients. Medication is not required; selection is individualized. Prescriptions, prior authorization, coverage, and results are not guaranteed. Medications are not included in program pricing, and WellifyCare does not dispense or sell medications directly.
Connected Obesity Care begins October 1, 2026 as a self-pay program. Virtual appointments are available to eligible adults physically located in Wisconsin. In-person appointments are available at the WellifyCare Medical office in Brookfield, Wisconsin. Program visits may be provided virtually or in person depending on patient preference, clinical appropriateness, and scheduling availability. Insurance-based obesity, diabetes, and cardiometabolic specialty care is planned for 2027. Participating plans will be announced after credentialing and contracting. The pathways remain separate; self-pay enrollment does not guarantee future insurance coverage.
This is a structured medical program, not a month-to-month subscription. Patients commit to the full six months. Time is needed to assess causes, titrate safely, evaluate response, build habits, navigate plateaus, monitor cardiometabolic progress, and prepare for maintenance. Cancellation and medically necessary early-termination terms are explained in the program agreement.
Who may benefit: adults in Wisconsin seeking physician-led obesity treatment, including those with weight regain, considering or currently using obesity medication, or managing prediabetes, type 2 diabetes, hypertension, abnormal cholesterol, fatty liver, elevated cardiometabolic risk, or sleep apnea risk, who are ready for a six-month program.
