These Website Terms of Use ("Terms") govern your use of wellifycare.com and the public website features operated by WellifyCare Medical S.C. ("WellifyCare Medical," "we," "us," or "our"). By using the website, you agree to these Terms. If you do not agree, do not use the website.

1. Emergencies

WellifyCare Medical does not provide emergency services. This website and its messaging channels are not monitored for medical emergencies. If you believe you are experiencing a medical emergency, call 911 or go to the nearest emergency department.

2. Educational Information; No Medical Advice

Website content is provided for general educational and informational purposes. It is not individualized medical advice and is not a substitute for evaluation, diagnosis, or treatment by a qualified health professional. 3. No Physician-Patient Relationship Through Website Use

A clinical relationship begins only after WellifyCare Medical accepts you for care and applicable registration, consent, evaluation, and other clinical requirements are completed.

Clinical services are governed by separate patient forms, consents, notices, financial policies, and care agreements. If those documents conflict with these Terms on a clinical-care issue, the applicable clinical document controls.

4. Eligibility and Telehealth

Services are subject to clinical appropriateness, scheduling, and legal requirements. Telehealth is available only when the treating clinician is licensed or otherwise legally authorized to practice in the state where the patient is physically located at the time of the visit. We may require verification of identity and location. An in-person evaluation or referral may be required when clinically appropriate or legally required.

5. Appointments, Fees, and Insurance

An appointment request is not confirmed until WellifyCare Medical communicates confirmation. Website descriptions do not guarantee eligibility, insurance coverage, authorization, reimbursement, medication approval, a particular treatment, or a particular result.

You remain responsible for confirming insurance benefits and for amounts assigned to you under applicable financial policies. Uninsured or self-pay individuals may have rights to a Good Faith Estimate.

6. Privacy

Our Website Privacy Policy describes information collected through the public website. Our Notice of Privacy Practices describes how WellifyCare Medical may use and disclose protected health information and explains patient privacy rights.

Any dispute concerning only the public website that is not governed by a separate clinical agreement will be brought in a court of competent jurisdiction in Waukesha County, Wisconsin, unless applicable law requires otherwise.

We may update these Terms. The current version will be posted with its effective or last-updated date. Continued use after an update means you accept the revised Terms. If a provision is found unenforceable, the remaining provisions remain in effect to the extent permitted by law.
