Comprehensive Evaluation
Your assessment includes two physician visits. The initial evaluation is approximately 45–60 minutes, and the results and roadmap visit is also approximately 45–60 minutes. Visit length depends on clinical need and complexity.

Dr. Dere synthesizes relevant testing and risk factors, including AHA PREVENT™ when applicable, to identify your most important next steps.

Testing when it is clinically needed.
Laboratory testing is billed separately. Patients may use insurance when applicable or choose available cash-pay laboratory pricing. Laboratory testing is individualized based on clinical need.
The assessment does not include lab charges, medication costs, imaging, sleep tests or equipment, CGM or devices, outside services, extra visits, ongoing remote monitoring, or emergency, urgent, or hospital care.

This assessment is for adults with weight concerns, prediabetes or type 2 diabetes, blood pressure or cholesterol concerns, liver, kidney, or sleep risk, family risk, or multiple abnormal numbers. Obesity is not required.
WellifyCare Medical S.C. is a specialty practice that complements primary care and specialist care. It is not an emergency service or a replacement for organ specialists.
Your roadmap may include Connected Obesity Care, focused cardiometabolic follow-up, primary care or specialist coordination, additional evaluation, or a self-directed prevention plan. There is no automatic enrollment. If follow-up care is recommended, we will explain the scope, schedule, and price before you decide whether to proceed.
Payment and Eligibility
Connected Cardiometabolic Assessment: $399 one-time. Laboratory testing and other third-party services are billed separately by the providing vendors. Insurance may be used when accepted and applicable; coverage is not guaranteed. The assessment may be eligible for payment using HSA or FSA funds. Patients should confirm eligibility with their plan administrator.
The assessment is currently offered as a self-pay service. Medicare and Medicaid beneficiaries should contact WellifyCare Medical before booking to confirm eligibility.
