High blood pressure, cholesterol, fatty liver, kidney risk, sleep apnea, and heart risk often travel together with weight and blood sugar. We evaluate them as one connected picture, sometimes called cardiovascular-kidney-metabolic (CKM) health, and build one coordinated plan.

Cardiovascular-kidney-metabolic (CKM) health describes the close link between metabolic health, kidney health, and heart disease. These risks often develop quietly and appear together — prediabetes or type 2 diabetes, high blood pressure, high cholesterol or triglycerides, excess abdominal weight, fatty liver disease (MASLD), protein in the urine or early kidney disease, or several borderline results without a clear plan.
Early identification and evidence-based treatment can help reduce future risk. We use CKM staging and the American Heart Association PREVENT™ calculator when appropriate, and your plan may include lifestyle treatment, medication, home monitoring, and coordination with your primary care clinician and specialists.

Home sleep apnea testing with WatchPAT® One

Home sleep apnea testing with WatchPAT® One
Untreated sleep apnea raises blood pressure, blood sugar, and heart risk, and makes weight loss harder. Many people with obesity, diabetes, or high blood pressure have it without knowing. WellifyCare Medical offers home sleep apnea testing in partnership with WatchPAT® One, a disposable home sleep test worn on the wrist and finger for one night.
How it works
Dr. Siyum reviews your sleep symptoms and risk during your visit.
If testing is appropriate, we order the WatchPAT One test.
The WatchPAT team checks your benefits, handles any prior authorization, and ships the test to your home.
You sleep in your own bed for one night, guided by a phone app — no sleep lab.
A board-certified sleep physician interprets the study, and Dr. Siyum reviews the results and next steps with you.
Treatment options may include CPAP, weight treatment, positional therapy, or referral to a sleep specialist. Tirzepatide (Zepbound®) is FDA-approved for moderate to severe obstructive sleep apnea in adults with obesity and may be considered when appropriate.
Billing: the home sleep test is billed to your insurance by the testing provider. Your WellifyCare Medical visit is billed separately. Coverage, prior authorization, and cost-sharing depend on your plan. Home testing is not right for everyone; some patients need an in-lab sleep study or referral to a sleep specialist.

Kidney function can change gradually and without symptoms, and early detection can change treatment. Heart risk reflects the combined effects of blood pressure, cholesterol, blood sugar, weight, kidney health, and other factors. Fatty liver disease often occurs with insulin resistance and may be silent. We evaluate these risks together as one connected picture.
